How to sign up
Thank you for choosing Hofmann Driving School!
The easiest way to register is to do it online here. The system will guide you through the entire process, pre-fill the required documents for you, and let us know that we should expect you.
If you prefer not to use online registration, you can find the necessary documents for printing and completion below.
Registration conditions
In any case, you must ensure that you meet the legal requirements:
✔ You meet the minimum age requirement
The required age depends on the category you are applying for. Training can begin even before reaching the required age (we recommend only a few months in advance), but you will only be able to take the exams after your birthday.
If you are under 18, a written consent from your legal guardian will also be required. There is no special form for this; it can be found in the registration form below.
✔ You are medically fit
You will prove this with the form below, which must be certified by your general practitioner.
✔ You have permanent or temporary residence in the Czech Republic
This can be easily proven with an ID card. Foreigners can use a confirmation of temporary residence or a long-term visa.
✔ You are legally allowed to drive
You must not be under a driving ban. This is confirmed by a sworn statement in the registration form below.

If you are registering online here, you can skip this part.
Otherwise, fill in only the first section of the form marked “VYPLŇUJE ŽADATEL”
Medical certificate
The form can be completed by any general practitioner, but we recommend the doctor with whom you are registered.
Attention! To avoid future issues, make sure that the stamp clearly shows the doctor’s name and their position as “general practitioner.” On the day of registration, the certificate must not be older than three months.
